You should supply original documentation required to support the statements on the application form. In addition to this please ensure that you fill in all the relevant sections of the application form. You should not send your passport at this stage.
It is extremely important that you read carefully the guidance notes on completing the form, and the evidence that should accompany your application, before completing the form. It is essential that you fully complete all the relevant sections of the form and supply the appropriate evidence to support any statements you make. Failure to do so may lead to a delay in the consideration or refusal of your application.
Principal applicant of HSMPAccording to the new rules of HSMP effective from 8th november 2006 the following documents are required for HSMP application.- Proof of age: birth certificate/ Original Driving License. You will also need to provide alternative evidence in the form of a notarised copy of the personal details page of your passport. (Please do not send your original passport with this application form).
- Educational qualifications - You should provide your original degree certificate. This should clearly show
- the date of the award; and
- the title of the award; and
- the name of the awarding institution; and
- your name.
Work experience certificates for all companies you have worked in. According to new rules of HSMP there is no requirement of experience certificates but it is better to attach it but remember now there are no points for experience.
Tax returns if you are claiming points for income. If the tax year does not cover the full period claimed other corroborating evidence is required for the period(s) not covered. Self-assessment tax documents are not accepted, as they are not independent.
Wage Slips if you are claiming points for income. Your wage slips covering the entire 12 months period.
For self employed individuals, we will need balance sheets, profit and loss statements, income tax returns for atleast 3-5 years. In addition, copies of client/ supplier references, contracts/ agreements are desirable
Spouse's Documents for HSMP According to the new rules of HSMP there are no points for the spouse qualification so there is no need to send these documents.
According to old system these documents wre required ONLY if you were claiming points for spouse skills for HSMP.- Proof of age : birth certificate/ school leaving certificate or passport
- Educational qualifications : degree certificate and mark sheets for all years
- Marriage certificate
- Proof that you are married for more than 2 years. Eg. telephone/ electricity bill, joint bank accounts, investments, society bill, property documents etc.
* The application form you will need to complete will depend upon your age group. If you are 28 years of age or over, you should apply on form HSMP 1. If you are under 28 years of age, you should apply on form HSMP 1(28).
* Passport of applicant or dependants are not required at the approval stage. These will however be required at the entry clearance stage (post approval of HSMP visa).
